Description
PHARMACIST SERVICES
1. The clinical pharmacist is responsible for planning, implementing, and operating the drug
monitoring and utilization programs in the outpatient areas. He/she is the expert in all areas
related to drug therapy and functions as a consultant to other health care professionals. He/she
will also be responsible for developing, providing, and supporting the educational programs of the
Pharmacy Service.
2. All orders (ex. VA and Community Care) are checked to include, but not limited to patient
information, correct medication, dose, route, and frequency of administration. Orders are checked
for patient allergies, drug interactions, incompatibilities, duplication, and priority.
3. Identifies, interprets, and resolves prescribing and dispensing issues, including
pharmacotherapeutics, drug interactions, contraindications, and formulary issues. Documents
interventions, clinical activities, and workload in a timely and professional manner, including
completion of all consults and encounter forms.
4. Processes controlled substances with accuracy according to current policy and procedures
outpatients.
5. Prescribed drug therapy is evaluated and communicated to physicians/pharmacists for
appropriateness, effectiveness, cost-benefit, adverse drug reactions, allergies, and disease
states.
6. Responsible for monitoring the hospital formulary, with particular respect to cost-effective
management of the use of available therapeutic agents. This includes the utilization of
computerized monitoring systems combined with incumbent clinical drug knowledge to constantly
monitor drug usage and make recommendations for formulary changes when deemed necessary to assure
optimal usage of drug resources.
7. Participates in the development and implementation of formal and informal drug
education/clinical consultation to medical, surgical, nursing, and other staff in the form of
inservices, one-on-one encounters, and continuing education programs. Serves as a resource to
patients, providers, and other pharmacy staff.
8. Effectively instructs patients and family members in the appropriate use of medications and
medical devices. Can adjust communication and tracking methods based on the population and
developmental considerations. Can accurately assess and document patient comprehension. Is
able to refer patients to educational programs provided at the medical center by a multidisciplinary
teams.
9. Routinely identifies the customer's/patient's situation properly and performs the tasks required
to resolve accurately and on time. Follows up, as necessary, to ensure a
satisfactory resolution.
10. Consistently responds to patients', families', visitors', and co-workers' requests for
assistance promptly and in a friendly and cooperative manner. Any deficiencies in this area are
limited and minor, and have no significant adverse impact on either the organizational or individual
productivity or efficiency.
11. Professional relationships with supervisors, coworkers, and others in the organization are
courteous and cooperative in nature, and, overall, contribute to the effective operations of the
workplace and the development of a collaborative working relationship.
12. Demonstrates the ability to adjust to change or work pressure in a pleasant and constructive
manner; handles differences of opinion in a businesslike fashion; and functions as an effective
team member through active participation in working towards team objectives and assisting in the
group effort.
13. Patient-specific medication-related requests, questions, or issues from other services (Nursing,
Medicine, Specialty, etc.) are handled professionally and accurately. Provides follow-up to ensure
a satisfactory resolution of any medication-related requests, issues, or questions.
14. The Clinical Pharmacist will conduct routine inspections of medication storage areas monthly and
provide documentation per local VA Pharmacy policy to the Clinic manager.
