Description
NURSE PRACTITIONER SERVICES

1.  Responsible for the provision of covered services to assigned and unassigned patients presenting for care, providing health care commensurate to the PCP's licensure and clinical privileges or scope of practice, ensuring the patient's care plan contains medical recommendations for clinically indicated care, offering clinically indicated health care services to patients assigned to the PACT, and providing or arranging for care to which patients consent, providing 
leadership to the team including shared delegation of appropriate care and care processes to appropriate team members, reviewing available clinical and performance data with the team, and focusing on continuous improvement of critical team processes, ensuring the patient has same-day access for face-to-face and virtual care visits during regular clinic hours, and collaborating with PACT staff to develop personal health plans that incorporate care management and care coordination appropriate to the patient's needs.

2.  Primary care includes but is not limited to medical diagnosis and medically necessary treatment(s) of physiological and pathological conditions not requiring referral to specialty care or in-patient hospital services. Care is directed toward health promotion and disease prevention, management of acute and chronic medical conditions, pharmacological management, referral to specialty care, and continuity across the inpatient and outpatient settings. Clinical assessment shall include assessment for military sexual trauma, potential exposure to herbicides, and other conditions specific to combat veterans.

3.  Nurse Practitioner shall provide women's health services, including but not limited to: annual PAP and pelvic exam per VA clinical standards; annual clinical breast exam and mammogram; family planning services; management of conditions related to menopause, including hormone replacement therapy when indicated, and other services as they pertain to the care of the female patient.

4.  VA requires access to the primary care provider within seven calendar days of the veteran's request for established patients and 20 calendar days for new patients.  Services are generally provided by appointment, but provision must also be made to accommodate urgent care needs of the patients enrolled at the CBOC.
