RN - Long Term Care

WJM Professionals is DESPERATELY recruiting Long Term Care (LTC) RNs for the VAMC in Richmond, VA.

 

The role of the Long-Term Care (LTC) RN is rewarding and impactful, knowing you are providing care and support to veterans.

 

It is a place where Veterans can receive the nursing home level of care, including help with daily living activities and skilled nursing and medical care.

 

THE RESPONSIBILITIES OF AN LTC RN ARE:

 

  • Performing assessments.
  • Administer medications and treatments.
  • Monitor vital signs and manage acute and chronic conditions. Provide wound care, manage catheters, and assist with activities of daily living (ADLs).
  • Provide resident and family education.
  • Manage admissions, discharges, and transfers.
  • Conduct comprehensive assessments of residents’ physical, emotional, and psychosocial needs.
  • Develop individualized care plans based on assessment findings.
  • Provide wound care, manage catheters, and assist with activities of daily living (ADLs).
  • Work closely with interdisciplinary teams, including physicians, social workers, and therapists.
  • Educate residents and families about health conditions, medications, and self-care.
  • Communicate effectively with residents, families, and caregivers.
  • Offer emotional support and address concerns.
  • Maintain accurate and timely electronic health records.
  • Ensure compliance with regulations and standards.

REQUIREMENTS:

  • Must be licensed as a registered nurse
  • Must possess an associate degree.
  • At least two (2) years of related experience
  • BLS certification is required.
  • Must have experience in geriatric or long-term care nursing.
  • Must maintain expertise in skilled nursing care, long-term care, and hospice and palliative care.
  • Will work with elderly and disabled Veterans, which requires empathy, patience, and understanding.
  • Proficiency in clinical skills such as wound care, medication administration, and assessment.
  • Proficiency in dementia care and managing complex medical conditions.

HOURS:

Services will be needed for all shifts in 24 hours/7 days per week/365 days per year, to include 8, 10, and 12 hours shifts as specified by the unit. Must be available to work every other weekend.
