1. Attend/complete all required training assigned by the Enrollment Supervisor and or Lead at VASDHS.

2. Process all applications for veterans requesting to be followed at the Contractor’s site.

3. Use a number of processes in making eligibility/priority group determinations including discharge
documentation, Veterans Information Solution (VIS), Health Eligibility Center Alerts, and
communications with the Eligibility Supervisor/Lead.

4. Contact the Eligibility Supervisor/Lead for any unusual or complicated enrollment issues/questions.

5. Adhere to the processes and guidelines established by the Eligibility Supervisor/Lead regarding all
issues concerning patient enrollment and registration.

6. All applications shall be registered and enrolled into VES. All registrations shall then be "Dispositioned"
in VISTA by using the "Disposition an Application" option and scanning of the 1010EZ including the
DD214 into the patient file before close of business each day. Any questions related to registrations,

7. Financial Assessments: Financial assessment is the process known as a Means Test (MT) that is used by
VA to assess the gross household income of Veterans who are not otherwise eligible for enrollment in
the EHR. The MT determines a Veteran’s copayment responsibilities, assists in determining enrollment
priority group assignment, and assists in determining beneficiary travel benefits.

8. For some veterans, an annual assessment of household income (and sometimes assets) shall be
completed by the veteran prior to being seen.

9. Provide a blank VA Form 10-10EZR (Renewal Application for Health Benefits) to the veteran; and the
veteran will fill it out completely, including the financial information on side two of the form. The
demographic and financial assessment information will be input into VISTA and maintained
